I authorize Kids House of Learning to care for my child during the time they are in the facility and to use all play equipment and participate in all rooms.

I grant permission for my child to be included in evaluations and pictures connected with the Center’s Program.

I give permission to Kids House of Learning to administer Tylenol to my child in the event that they begin running a fever while in our care. I understand that I will also be notified by telephone in this event.

I grant permission to administer first aid in the event of a minor accident or injury.

I grant permission for my child to hold their bottle while laying down in crib.

Yes

I grant permission for my child to participate in water activities, I will be notified.

Yes

I grant permission for the Director or Acting Director to take following steps in the event of any emergency medical care, if warranted.

Yes

1. Attempt to contact a parent or guardian
2. Attempt to contact the child’s physician.
3. Attempt to contact the parent through persons listed on the emergency list.
4. If we cannot contact you or the child’s physician, we will do any/all of the following.
a. Call another physician
b. Call an ambulance
c. Have the child taken to emergency room at the hospital in the company of a staff member.
d. Notify the Department of Health and Hospitals if (c) is used.

Kid’s House of Learning will not be responsible for anything that may happen as a result of false information given at the time of enrollment.

All children shall be escorted in the building and signed in and out.

Yes

All parents will be responsible for checking their children in/out properly. If on child care and we do not receive payment from them, you will be responsible.

For services listed in the handbook, I agree to pay weekly tuition and fees.

I agree to cooperate with the general policies as outlined in this Manual and to perform the obligations as parents/guardians as set forth. My signature below indicates that I have received and read the terms listed in this handbook entirely and have had all my questions answered.
